you are enrolled in Heartburn Health. Heartburn Health will only receive the
information you share in the enrolment form. The company who sent the text
message on our behalf will delete your information in 30 days.
If you do not want to join the programme, please ignore the invitation. Please do
not complete the withdrawal form if you have not enrolled.

What are your options?
Leaving the programme will not impact your care or legal rights in any way. You will not need to give a reason for leaving.
You have two options:
- Stop future contact from Heartburn Health, but allow the programme to continue collecting health information from your NHS records
- Stop future contact from Heartburn Health and stop us collecting health information.
We will keep any information about you that we already have. This is to make sure that our research is reliable.
